Covid Health Notification

Good Morning WYHA Families,

Hope that you are well. As we are about to enter another weekend of hockey, we wanted to take a moment to provide a health notification and review our Covid-19 precautions that we must all adhere to in order to deliver a safe and healthy hockey experience for all members of Wilmington Youth Hockey and adhere to guidelines outlined by the state, VHL and rinks.

During this past week, Covid-19 has impacted our Bantam program. The Bantam players experienced a direct contact exposure with an individual that later tested positive for Covid-19. Once notified, WYHA began working directly with the Wilmington Board of Health and supported the contract tracing efforts that were done by the Board of Health. The Board of Health provided guidance to the families of those direct contacts, as well as directed WYHA to pause play for the Bantam Teams for 14 days. 

Please join us in wishing all of our teammates, families and coaches impacted by Covid-19 all the best as they move through this stressful time. 

It is critical that we strictly adhere to the Covid-19 protocols set forth by the State of Massachusetts, Valley Hockey League and Rinks as well as the WYHA Health Attestation. If we don't, our ability to continue our hockey season will be in question. Please review the Health Attestation below. Self-administering this attestation prior to attending all WYHA activities will keep your team, coaches and hockey family healthy. If you are sick - stay home. If you have had an exposure - stay home. If you are unsure if you should attend after completing the Health Attestation - please reach out to your medical provider or the Board of Health for guidance. 

Thank you for your partnership and cooperation in delivering a healthy and safe hockey experience for all. We all play an important role in making that happen. 

Player Health Attestation – Parent/Guardian MUST self-administer the Health Screen below for your player prior to attending ALL WYHA Activities.

If your player is unable to pass the health screen due to symptoms – please remain out of ALL WYHA activities until the symptoms abate.

If you answer yes to a question around Exposures to Covid-19 – you must remain out of WYHA activities for 14 days or until cleared to return by Wilmington Board of Health.

Screening Questions:

Parent/Guardian will review the following questions and any player answering “YES” to ANY question will NOT be permitted to participate in the WYHA activity.

 

      In the last 72 hours has your player had any of the following symptoms?

Fever 100.4 or higher(or would have but for use of fever-reducing medicine) or chills?

 

 

Cough?

 

 

Sore Throat?

 

 

Muscle Aches?

 

 

Difficulty Breathing?

 

 

New loss of taste or smell?

 

 

EXPOSURE - In the last 14 days, has your player or anyone in your household:

Had a confirmed case of COVID-19?

 

 

Had close contact with anyone with a suspected or confirmed case of COVID-19?

 

 

Been advised or directed to quarantine or self-isolate due to COVID-19?
